Chapter Proxy Forms

Fellow Warrants,

There are times when we cannot make it to our regular meetings; this is understandable. But, even without you, the business of the Chapter must go on. So, as a safety valve for those times when you cannot make the meetings, USAWOA has prepared a Chapter/Region Proxy form that members can execute and provide to their local chapter or region.

The proxies are used to execute your desires as if you were at the meeting in person.

Why File a Proxy?

Ensures your voice is heard even when you can't attend
Allows voting on chapter business and elections
Maintains your active participation in chapter decisions
Simple to complete and valid for extended periods

How to Submit Your Proxy

I would like to request that each member execute a Chapter Proxy that will be kept on file with the Chapter Secretary.
